Metal on metal (MoM) resurfacing and total hip replacement
The Cornwall Hip Foundation team have used metal on metal hip implant bearings with resurfacing hip replacement since 2002. From 2004 onwards, MoM total hip implants were used. Some metal on metal implants have now been noted to cause severe allergic reactions in the soft tissue around the hip resulting in pain, swelling and weakness of the hip due to damage to the muscles around the hip.
The ASR hip and the Durom hip have caused more problems than other models used. All patients with ASR implants have been identified and recalled already.
The most common resurfacing implant used by the Cornwall Hip Foundation has been and remains the Cormet from UK manufacturer Corin which seems to have caused far less of a problem than many other models.
All patients with resurfacing implants in place should be under annual review and any problems with the hip will have been looked into if necessary.
Patients with total hip replacement MoM bearings have usually been discharged from follow up.
The implants were used predominantly in younger, more active patients. They are unlikely to have been used in patients over 75 years of age.
These patients are being traced on our system and will be contacted by post with a questionnaire about their hip. If the hip is functioning well and is entirely pain free, there is nothing else that needs to be done at this stage.
If however the hip is not quite perfect, we will be arranging an MRI scan to assess for any soft tissue swelling or fluid collections. If a hip is found to have a problem, we will be recommending that the bearings are changed. This will obviously require surgery.
Any patient having surgery since the start of 2011 will not have had a MoM bearing.

Since 1962, when Sir John Charnley developed the first successful hip replacement, British Orthopaedic surgeons have increasingly relied on replacement of the hip joint with an artificial joint to treat patients with pain.
In the last decade however, there has been increasing interest in techniques designed not only to treat pain from the hip, but also to attempt to correct the underlying cause of the pain. There is an increasing but as yet unproven belief that early treatment and correction of mechanical problems may delay or protect the hip from developing hip arthritis.
These techniques may also delay the need for hip replacement surgery but as yet, this is not able to be proven.
Results presented from the Cornwall Hip Foundation surgeons experience of their initial experience of over 140 cases of surgically treated patients with hip pain are encouraging and as techniques of diagnosis and surgery improve, it is likely that the results will improve also.
Initially, hip preserving surgery was only undertaken by opening the hip and formally taking it out of joint to repair and shape, but as techniques, knowledge and equipment has developed, the use of hip arthroscopy has overtaken this form of surgery in many instances.
