Cigna has very affordable copay plans.  They accept or decline, so if they accept you they include all pre-ex conditions.  They may increase premiums to cover the pre-ex.

Humana has the most affordable high deductible plans for the HSA.  You may want to consider the plan that does not include Rx- big savings.
