Alert for Newton, Wellesley & Weston families
that have had a child diagnosed with type 1 diabetes:
In May 2009, school nurses in Newton, Wellesley, and Weston invited families that have had a child diagnosed with type 1 diabetes to participate in a unique Massachusetts Department of Public Health (MDPH) investigation being conducted in collaboration with the Joslin Diabetes Center.
Please be sure the MDPH includes your family in this study, if you have a child who meets the following criteria:
Diagnosis: type 1 diabetes
Born: up to 19 years ago
Residence: Newton, Wellesley, or Weston
MDPH will include all children, regardless of where they have received diabetes care.
MDPH will include your family’s data when you submit a simple 2-page form. Click on the following links to download:
